Welcome to The College of Wooster! As a sports medicine staff, we strive to provide the best possible medical care for our student-athletes. Our staff of physicians, athletic trainers, and wellness center staff will help keep you healthy and when needed, heal your injuries with all the resources available to us.  In order to take advantage of these services and fulfill NCAA requirements we need to update your account in the Student Health Portal. You can do this by following the steps below. Please sign into the College of Wooster Student Health Portal to complete all forms and upload all relevant information by July 31, 2025. All student-athletes MUST complete all forms before you will be cleared to participate in ANY organized team activities.

How to log in and complete forms:

Log into the Student Health Web Portal at https://lwc-portal.wooster.edu/

Enter your College of Wooster credentials (username/password) and complete the 2-factor authentication process to finish the login process.

Click on the tab along the top of the screen titled My Forms

You MUST fill out all the following athletic forms before you are allowed to participate in your sport:

• Insurance Plan Information
• Athletic Insurance Information and Procedures
• Concussion Statement Form
• Sickle Cell Trait Information Waiver (first-year varsity athletes only)
• NCAA rules require all student-athletes to know their sickle cell trait status
• Student Release of Information
• Varsity Sports Waiver

You must also complete a physical examination and mental health screening prior to participating in your sport. Both processes will be completed on campus at the Longbrake Student Wellness Center, you do NOT need to schedule a physical prior to arriving on campus. A schedule of these times will be sent to you in August prior to your arrival on campus. If you are unable to make your scheduled time, please contact damoore@wooster.edu to reschedule.
